In order to best help you, we may collect the following personal information that you voluntarily provide: Name, Date of Birth, Address, Telephone Number, Email address, Insurance information, Other Family Members' Names/Information.  This is NOT an all-inclusive list as our intake forms are designed specifically for different appointment types. 

Midwest Family Eyecare handles all protected health information in accordance with laws, including the Health Insurance Portability and Accountability Act (HIPAA).  We do not sell information. 

Remember, we use this information safely to do a variety of tasks - respond to inquiries, communicate with you, schedule appointments, process insurance information, etc.  We use an electronic medical record platform as well as a patient communication system to help us complete this tasks. Your information may be shared with other healthcare providers involved in your care as well as with your insurance provider for claim processing. 

If you have a cell phone or email address, we utilize these forms of communication for - recall notices, communications, appointment reminders, and more. You may OPT OUT of these for non-essential communications during your completion of our online paperwork prior to your appointment. 

Our website uses analytics which may contain the following information: IP address, device information, pages viewed, time and date of website visits, etc. It may also use cookies and similar technology. This is to improve function of the website, analyze traffic on our webpages, and enhancing user experience. Cookies CAN be disable but it may disrupt the function of some website features. Midwest Family Eyecare uses reasonable technical and physical safeguards to protect your information from unauthorized access, disclosure, alteration or destruction. Unfortunately, no method of electronic transmission or storage is 100% secure; therefore we cannot guarantee absolute security. If there is any breach, we will promptly notify you.
